Through California’s CalAIM initiative, CommUnify is expanding care coordination for Medi-Cal members with complex needs. Enhanced Care Management gives eligible members a dedicated care team to coordinate physical health, behavioral health, and social needs in one place — alongside Community Supports that address real-life barriers like housing and food.

We’re growing these services to reach new populations across the county in the months ahead.

Repairs focus on health, safety, and fall prevention. Some services we offer are grab bars, handrails, handicap toilets and ramps. We also connect seniors to community partners for continued care.

No-cost home energy upgrades and education — designed for safer, healthier, more energy-efficient homes that cost less to run. Home safety testing, Energy Star appliance upgrades, air sealing and insulation, and heater or hot water repair or replacement.
